FAIR USAGE TERMS

Square Health reserves the right to deny service and/or to terminate the account of an end-user in the following circumstances:

• Seeking to book an unusually high number of consultations within a specified period
• Seeking to book a consultation for the same, or substantially the same condition, in circumstances where a practitioner has advised that no treatment, or no further treatment is available within the service and/or has recommended specialist treatment outside the service
• Failure to disclose material information that should reasonably be provided to a practitioner to facilitate diagnosis and/or treatment
• Unreasonable refusal to allow health or medical information to be shared with GP or other relevant practitioner
• Using, or attempting to use the service for the benefit of any third party not covered under the service
• Sharing login or other confidential credentials with any third party
• Abusive conduct towards any Square Health staff
• Other conduct which Square Health reasonably determines to be a breach of its usage terms or in bad faith
